By submitting this form, I understand that I am responsible for all library materials borrowed on my library card and agree to pay any charges when material is damaged, lost or returned late; and to notify the library immediately upon change of address, email, or phone number. I also understand I will not be able to check out library materials with a hold on my account.

After Submitting This Form

After submitting your application, please bring a valid photo ID and proof of current address to the library to retrieve your card number. If you are out of the county, your card will not be enabled until the library card payment ($15) has been made.
