Mid-Term Exam Transportation Form

All Upper School parents (not needed for Middle School) please complete this form so that we can ensure proper staffing of study hall rooms on Monday and Tuesday exam days. (Note: Friday is a half day for ALL students)

Please note: all students must either leave the campus after they have finished their exam(s) or go to an assigned study hall until they are picked-up.

This form must be submitted by January 18, 2023.
