NOTE: If this is about a misunderstanding of your officiating history, please reach out to certification@wftda.com to discuss the issue. If no resolution can be reached, they will direct you to the correct form when they have determined that this is your final option. Sending an email is usually much, much faster.

1. Initial review does not contain enough information regarding why I was not granted the level for which I applied, and/or there is no actionable feedback for me to work on to improve.

2. The Certification panel weighed certain information disproportionately when determining my overall qualifications.

3. I believe there is a conflict of interest and/or bias in the Certification Panel that performed my review.

4. There is incorrect information in my file, which should not be considered in the decision.

No new information is allowed from others in your Appeal application. However, you may use this space to provide your own self evaluation regarding your concerns.
