I will pay monthly fees prior to first day of class.

Liability Release: I am aware that dance training and the athletic exercises associated with it place unusual stress on the body and carry the risk of physical injury. On behalf of my child and myself, I assume the rest and agree that Ashland Danceworks Inc. shall not be liable in any way for injuries sustained during attendance at the dance studio, virtually or any of its related functions.

Publicity Release: I hereby authorize Ashland Danceworks Inc. to record the student’s picture and voice on photographs, films and tapes, to edit these recordings at its discretion, and to incorporate these recordings into movie and sound films on tapes, radio or television broadcast programs. Ashland Danceworks Inc. is permitted to use these materials for publicity, advertising and sales promotion and to use the student’s name, likeness and voice and biographic or other information in connection with them. I acknowledge that Ashland Danceworks Inc. made no promise of compensation for such use.

Session Fees, Withdrawal and Cancellation Policies: I understand that enrollment is for the 2020 Fall Virtual Classes. I understand that dance classes offered at Ashland Danceworks Inc. are non-refundable and payments will not carry over. All fees must be paid in advance to participate in the classes. Ashland Danceworks is not responsible for family cancellations and or illness. In the event there is an additional Government orders to close businesses, refunds will be issued.
