This form is for contacting the CRC if you feel a captain, teammate, or team server has violated the NLTP code of conduct but you are not comfortable speaking up about it. Names are required for accountability but under no circumstances will your name be released publicly or to any members of the community not currently on the CRC. Future and past CRC members do not have access to this. A new form is created for every season.
