Final Evaluation: TSP May 21 cohort
This is an evaluation form to collect and gather feedback on the Textbook Success Program, particularly your experience in Phase 2 (last 9 months). Your responses will remain confidential, viewable only by Rebus staff. Responses can remain anonymous should you choose. The survey should not take longer than 15 minutes to complete.

We appreciate your input as it helps us to:
1. Gather information on the value and impact of the program
2. Improve the program's efficacy — be it relating to the curriculum, structure, or tools.
3. Determine your interest in being a TSP facilitator

We believe it's important for potential program participants to hear directly from alumni and graduates, so public reviews about the program will be shared on the Rebus Community website, and in any marketing materials about the program. As stated above, other feedback provided in the survey will remain confidential.
