REMINDER: The cost of US Lacrosse Membership is now included in our registration fee! You do not need to register with US Lacrosse. US Lacrosse membership numbers will be distributed to players at the start of the season. (If you have a US Lacrosse number this will add a year onto the membership or renew it)

Online Registration dates & fees:
November 15- February 15: $125.00 (Early Bird)
February 15 - first game: $135

For players who registered last year and paid their dues and uniform fees, no balance due.

Season Runs March - June  We anticipate beginning practice March 1st as per League rules

YOUTH LACROSSE RELEASE   I do hereby fully release and discharge West Milford Youth Lacrosse, and West Milford High School Booster Club, parties to be jointly known in this agreement as West Milford Lacrosse., its officers, directors, coaches, assistants, referees and other agents or persons engaged in the activities of West Milford Lacrosse, from any and all claims for injuries, damages or loss which I may have or which may occur to me on account of my child’s participation in this program.I further agree to indemnify and hold harmless and defend West Milford Lacrosse,. and its officers, directors, coaches, assistants, referees and other agents or persons engaged in the activities of West Milford Lacrosse, from any and all claims resulting from injuries, damages or loss sustained by anyone, and arising out of, connected with or in any way associated with my conduct and the activities of this program.I further understand that terms such as “participation”, “program” and “activities” referred to in this release include all exercised and physical movements of any nature while I am participating in this program and further include the provision of or failure to provide proper instruction or supervision, the use and adjustment of any and all equipment, machinery, apparatus, and anything related to my use of the services, facilities, or premises involved in this program, and transportation to and from any events.I understand the nature of the program for which I am registering, and have read and fully understand this Waiver and Release.  I further understand any advisement or warnings of the particular risks of this program and I therefore acknowledge that they are part of this Waiver and Release.                          MY CHILD IS WELL AND ABLE TO PLAY THIS SPORT.  I WILL ADVISE THE COACH OF ANY CHANGES IN MY CHILD’S HEALTH STATUS.                                      In addition to the parent/guardian permission and waiver for my child’s participation in any and all activities and events associated with West Milford Lacrosse to the fullest extent permitted by law, the undersigned agrees to indemnify and hold West Milford High School Booster Club, its elected officials, employees, and volunteers and others working on behalf of West Milford Lacrosse, and the organization named above, its officers, directors, managers, coaches, officials, and other agents or persons engaged in the activities of the organization named above harmless from and against all loss, cost, expense, damage, liability or claims, whether groundless or not, arising out of the bodily injury, sickness or disease (including death resulting anytime there from) which may be sustained or claimed by me/my child or the damage or destruction of any property, including the loss of use thereof, based on an act or omission, negligent or otherwise of the undersigned, or anyone acting on my behalf in connection with or incident to the organization named above activities. I further agree to indemnify and hold harmless and defend the organization named above and its officials, directors, managers, coaches, officials, and other agents or persons engaged in the activities of the organization named above from any and all claims resulting from injuries, damages, or loss sustained by anyone, and arising out of, connected with, or in any way associated with me/my child’s conduct and the activities of this program. I further understand that participation and activities include all exercised and physical movements of the nature while I am/my child is participating in this program and further include equipment, machinery, apparatus, and anything related to my/my child’s use of services, facilities, or premises involved in this program, and transportation to and from any events. Code of Conduct I pledge to be responsible for my words and actions while attending, coaching, or participating in a youth sports event and shall conform my behavior to the following code of conduct: 1. I will not engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee. 2. I will not encourage my child, or any other person, to engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee. 3. I will not engage in any behavior which would endanger the health, safety or well being of any coach, parent, player, participant, official or any other attendee. 4. I will not encourage my child, or any other person, to engage in any behavior which would endanger the health, safety or well being of any coach, parent, player, participant, official or any other attendee. 5. I will not use drugs or alcohol while at a youth sports event and will not attend, coach, officiate or participate in a youth sports event while under the influence of drugs or alcohol. 6. I will not permit my child, or encourage any other person, to use drugs or alcohol at a youth sports event and will not permit my child, or encourage 7. any other person, to attend, coach, officiate or participate in a youth sports event while under the influence of drugs or alcohol. 8. I will not engage in the use of profanity. 9. I will not encourage my child, or any other person, to engage in the use of profanity. 10. I will treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability. 11. I will encourage my child to treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability. 12. I will not engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee. 13. Written warning issued by a league or organization from Youth Lacrosse League Board of Directors. 2. Suspension or immediate ejection from a youth sports event issued by Youth Lacrosse League Board of Directors. 3. Suspension from multiple youth sports events event issued by Youth Lacrosse League Board of Directors. 4.
