Tuition Subsidy Request form for Terms 5 and 6 2024  ONLY
This form is for Wiltshire schools to request subsidy for disadvantaged and vulnerable pupils' instrumental/vocal music tuition if they are new to the scheme for TERMS 5 and 6 of 2024 only. 

The subsidy scheme can only support a child learning ONE instrument - our funds cannot stretch to support children who wish to learn multiple instruments. 

For tuition taking place in school we can offer subsidies for a maximum of 10 lessons over the course of terms 5 & 6.

In order to complete this form you will need to know:
  • That the tutor is an Associate of the Hub - you can check here.
  • How much the Associate charges per hour
  • How many sessions the Associate will teach the pupil during terms 5 & 6 2024
  • How many pupils the Associate is teaching
  • The length of each pupil's lessons and whether they have individual or shared lessons
  • Whether the pupils taught are eligible for Pupil Premium/Free School Meals, have Special Educational Needs and/or a Disability (SEND), or BOTH
Subsidy Caps:
For the Academic Year 2023-24 there is a cap on the amount of subsidy that each school can access for tuition. For primary schools is the cap is £2,000 and for secondary schools and special schools the cap is £4,000.  

If you have already reached the cap for this academic year then you can only apply for the following additional amount for Terms 5 & 6: 
  • £300 for primary schools
  • £500 for secondary schools and special schools
If you make requests that go over this limit they will not be approved.

Deadlines:
The deadline for applying for terms 5 & 6 subsidy is Friday 17th May 2024. 
 
Payments:
Subsidies will be paid ASAP after the requests have been approved. We trust this enables Associates to invoice for the work ASAP to support them with their continuing work in your school.

If a request is not completed in full or incorrectly, the school will be asked to update edit and resubmit the form.

The person completing this form will receive an email outlining the  request, and with a link to edit it should it be required. This will come from a gmail account, therefore please make sure your school servers allow incoming mail from gmail.
