KUR DJ Services Request Form
PLEASE READ BELOW

A few important notes about requesting KUR services for an event:
1) We will need 3 weeks notice for an event to be considered.
2) We may or may not be able to help with your event. We will let you know 2 weeks before an event.
3) We require electricity, a table, and chairs for an event.
4) We require advance notice if you have any groups performing at the event, and we require any music for performances at least a week ahead of time.
5) We require a full schedule of any activities or performances throughout the event.
6) Donations to KUR, while not required, are encouraged.
7) We will try our best to accommodate any special requests and communicate with the organizer about what we can and cannot do.

If above answer is yes: Please specify how you will send any music or audio for performances. Songs must be pre-edited by the performing group and sent to KUR at least 1 week prior to the event – KUR will not be responsible for figuring out when to start or stop a song. If KUR is not provided with performance songs or audio prior to the event, we will not be able to DJ for the performance.

Would you consider a small donation to KUR for doing your event? (Note: Donations are not required and answering "yes" or "no" will not change your chances. We are happy to help organizations no matter what. Answering this will simply prompt us to follow up with you after the event to see if you'd still be willing to donate a small amount if you were satisfied with our work).
