SEHS Chromebook Request Form
This request form is for students who will be attending face-to-face classes at the SEHS campus this year and are in need of a Chromebook/laptop to bring to school on a daily basis. If a student already has a personal Chromebook or laptop that can be brought to school on a daily basis, parents should NOT complete this form. Students who already have Chromebooks or laptops are encouraged to bring their own devices to school. Tablets, iPads, cell phones, Kindles, etc. cannot be used in the place of a Chromebook or laptop.
Parents who complete this survey by August 3rd can expect their students to receive a Chromebook during the first three days of school. Information in the survey must be accurate so that it can be verified by school officials. You may be contacted to verify information prior to your student receiving a Chromebook/laptop. This form is only for SEHS students who are attending face-to-face classes. Be sure to only submit one form per SEHS student; multiple forms for one student will slow the distribution process. If you have more than one student at SEHS, you need to submit one form for each student who needs a Chromebook/laptop.
In completing this request and receiving a Chromebook/laptop for your child, you agree to maintain the Chromebook and return it to the district in the same condition in which it was received. You also agree to be charged $275 dollars for a device if it is lost, stolen, or damaged beyond repair. Furthermore, a cost of $50 will be assessed to cover repairs for broken keyboards, screens, or charging cables.
