I, Chrystal Chao Ci Xin would appreciate your assistance with this research project on the status of a disabled person’s quality of life. This research will help me understand the quality of life of disabled people in Malaysia as well as procedure we can do to help them improve their quality of life. All you need to do is complete this short questionnaire, which should take approximately 5 to 7 minutes. If you do not wish to participate, simply discard the questionnaire. Responses will be completely anonymous; your name will not appear anywhere on the survey. Completing and returning the questionnaire constitutes your consent to participate.
