Students: You will need to fill out this form prior to joining your first group, or, after a significant break.  IF ANYTHING NEW HAPPENS, please let us know by email as soon as possible. Please fill out as accurately as as you can, someone will get back to you in 1 business day :-)

This waiver acts as part of our screening processes which assures we are accepting clients that can safely participate in person once the COVID-19 crisis subsides. Every participant is required to complete at least one private training session with Mindful Mutts prior to joining at our professional rates. Once we receive this form you will be contacted to schedule this session! All RCRD Club gatherings via zoom are held every other Sunday morning, and are currently FREE of charge! Once quarantine is lifted and we are all safe and able to meet in person for practicing skills we will be requiring a donation of a min. $10 to the New Westminster Animal Shelter.

By filling out and submitting this form you understand that there are no refunds or credits, ALL training is non-transferable.

*Royal City Reactive Dog Club is held by Mindful Mutts. We reserve the right to decline registrations or make mandatory one private session with a science-based traiber prior to group. Instructions for attending will be very specific and MUST be adhered to at all times for everyone's safety (humans, doggos, community.)

While attending group, if we feel your dog is not coping well, or the public is at risk, you will be instructed to leave and to follow up with the trainer.   *
The trainer may further refer you to a Veterinary Behaviourist to consult with you and your veterinarian via email. This may mean pausing Reactive Dog Club participation temporarily or suspending altogether. *
You will be required to bring with you to class the following items (mandatory): Dog will be on well fitted front clip harness, 6 ft leash (no retractables), treat pouch, VERY HIGH VALUE FOOD (prepared into small piece before class- cheese, sausage, cooked chicken, tuna fudge, hot dogs etc), a toy attached to a 6 foot line (not thin string, not heavy rope, a puppy leash is great! ) If you have any questions or need to purchase an item prior to class, please email info@mindfulmutts.ca and we will arrange item/payment prior to class. *
If you have a gift certificate, please enter the gift certificate number below. Physical copy of GC MUST be brought with you and handed in at the start of class. Expired certificates will require payment in order to participate. *
