This is the official enrollment form for the Dragonfly Learning Center. By completing this form you are letting us know that you would like to enroll your child at the Dragonfly. By completing this form you are securing your child's spot at the Dragonfly. Enrollment is dependent upon the completion of all forms, submission of the registration fee, and the compatibility between TDLC and my child. Your spot at TDLC will be held for ONE WEEK after initial contact is made. If we have not heard from you within this time frame, your spot may be given to a different student. Please be sure you are actively communicating with our staff to ensure your spot is saved.
Initial and continued enrollment will be at the discretion of The Dragonfly Learning Center based upon the best interests of the child, the expectation that he/she will benefit from the program and the welfare of the other enrolled children. TDLC does not discriminate on the basis of a person’s race, creed, sex, age, religion, national origin or disability when determining enrollment eligibility.
