COVID-19 Violation Report
KHSTA is dedicated to making and keeping our workplaces safe for ALL staff members and students. There are specific guidelines that all workplaces in California are mandated to follow, which include: wearing of masks indoors and ensuring that all employees are trained in and following the California Department of Health and Kern County Public Health Department guidelines.

In order to ensure the safety and well-being of every employee, these guidelines must be followed. Regardless of personal opinion, this is a state mandate. The intent of the form is not punitive. In order to ensure guidelines are being followed, it is important for information to be in real time (a complaint about a work site that is not following guidance on Friday will not help ensure compliance if the violation occurred on the previous Monday) and be as specific as possible. Nothing can be done to correct improper safety protocols if administrators don't know who isn't following the guideline (for example, if you report "masks aren't being worn" that cannot be fixed with anything other than a general email).

This form is ANONYMOUS. No emails addresses are collected and this form can be accessed from work or personal email addresses. KHSTA will share only site location and details needed for the district to follow up and correct the issue.
