YNE Hockey League Selection Exemption Pass Request Form
Requests need to be sent by 2100 on the Wednesday before the Saturday game to which they apply.  Emergency/retrospective requests can be sent after, but the ALMC will not grant a retrospective SXP for an issue that could have been raised in good time.

Please ensure to complete the form fully & accurately, SXPs give on the basis of inaccurate information are not valid.  Requests might be rejected on the basis of missing or incomplete information.

Requests are usually processed on Thursday & Friday afternoons, allow at least 48 hours for a response.

You should submit one form for each player for whom an SXP is required (except where there's an option "Player 2 Name").
