SECTION G: PARENTAL/CARER/LEGAL GUARDIAN CONSENT
My child is in good health and I consider him/her to be capable of taking part in weekly training. I have completed the medical information as listed above and I understand that the information will be shared with other relevant members of the Club, e.g. Coaches. I consent that in the event of an emergency any necessary treatment can be administered to my child, which may include the use of anaesthetics.  I understand that in such an occurrence all reasonable steps will be taken to contact me or the alternative adult named above.  I also understand that Football can be potentially hazardous and while Coaches and assisting personnel will take every precaution to ensure that accidents do not happen, they cannot necessarily be held responsible for any loss, damage or injury suffered by my child.  By returning this completed form, I agree:

1. to my child taking part in the activities of the Club and agree with measures the Club has in place to manage risk, including its COVID-19 mitigations in line with current FA and Government guidance;
2. that I have read and agree to adhere to the Club Code of Conduct for Parents/Carers and Spectators; the Health & Safety Policy; and am aware of the Club Constitution (to be found at www.southdownfc.co.uk). I understand and agree to the responsibilities which I and the child named above have in connection with these policies.
3. neither the Club nor Committee shall be liable for any loss, damage or injury to person or property, however arising;
4. that membership fees become payable at the start of each season and that these fees are non-refundable even if my child ceases attending Southdown FC part way through a season (applicable to League players only).
5. I give my consent to Southdown FC, or a photographer appointed by the Club, taking and storing photographic or video images of my child’s involvement in football for the purposes of publicising and promoting the Club or sport, including use on the Club’s social media (website, Facebook page/group etc.) or as a coaching aid.
6. I am responsible for settling any fines from leagues due from bookings or sending off during matches.  I confirm that the ownership of all kit/equipment/jackets etc. remains the property of Southdown FC and the annual fee does not constitute ownership of these items.
7. I give my consent for personal information provided to or collected by Southdown FC, and that of my child, to be held, processed, shared, stored and destroyed in accordance with Southdown FC's Privacy Notice and Data Protection Policy (documents to be found at www.southdownfc.co.uk).

ANTI-BULLYING – STATEMENT OF INTENT - We are committed to providing a caring, friendly and safe environment for all of our members so they can participate in a relaxed and secure atmosphere. Bullying of any kind is unacceptable at our Club. If bullying does occur, all players or parents should be able to tell and know that incidents will be dealt with promptly and effectively. We are a TELLING club. This means that anyone who knows that bullying is happening is expected to tell the Club Welfare Officer or any Committee Member.

SOUTHDOWN FC CODE OF CONDUCT FOR YOUNG PLAYERS (Please click I agree after your child has read and understood the code of conduct for young players) *
We all have a responsibility to promote high standards of behaviour in football.  As a player, you have a big part to play.  At Southdown FC we ask every player to follow the FA's RESPECT Code of Conduct. When playing football, I will: • Always play to the best of my ability and for the benefit of my team • Play fairly – I won’t cheat, complain or waste time • Respect my team-mates, the referee, the other team, coaches & managers, officials and spectators • Play by the rules, as directed by the referee • Always respect the match official’s decisions • Be gracious in victory and defeat - I will shake hands with the other team and referee at the end of the game • Listen and respond to what my coach/team manager tells me and understand that a coach has to do what is best for the team and not one individual player • Talk to someone I trust or the Club Welfare Officer if I’m unhappy about anything at my club. I understand that if I do not follow the Code, any/all of the following actions may be taken by Southdown FC, the League, County FA or The FA I may: • Be required to apologise to my team-mates, the other team, referee or team manager • Receive a formal warning from the Club • Be dropped or substituted • Be suspended from training • Be required to leave the Club. In addition, Southdown FC, A League, County FA or The FA may make my parent or carer aware of any infringements of the Code of Conduct.  The FA/County FA could impose a fine and suspension against my Club.

We all bear a collective responsibility to set a good example and help provide a positive environment in which children can learn and enjoy football.  Play your part and observe the FA's RESPECT Code of Conduct at all times. I will •Remember that children play for FUN • Applaud effort and good play as well as success • Always respect the match officials’ decisions • Remain outside the field of play and within the Designated Spectators’ Area (where provided) • Let the coach do their job and not confuse the players by telling them what to do • Encourage the players to respect the opposition, referee and match officials • Avoid criticising a player for making a mistake – mistakes are part of learning • Never engage in, or tolerate, offensive, insulting, or abusive language or behaviour. I understand that if I do not follow the Code, any/all of the following actions may be taken by Southdown FC, the League, County FA or The FA I may be: • Issued with a verbal warning from a Club official • Required to meet with the Club Disciplinary Committee • Required to meet with the League or County Welfare Officer • Obliged to undertake an FA education course • Obliged to leave the match venue by the Club • Requested by the Club not to attend future games. In addition, the FA/County FA could impose a fine and/or suspension on The Club.
