As my dog is going to be in contact with live and dead birds, I understand the importance of having my dog on an appropriate deworming schedule and will have them dosed as recommended by my vet.

I confirm that my dog has complete and updated vaccinations per the AAHA Canine Vaccination Guidelines. I will provide vaccine records to Westrock Kennels. I understand that while my dog is fully vaccinated, vaccines are not guaranteed and there is a small risk that my dog may contract a contagious disease or illness while boarding at Westrock Kennels. I agree that should this occur I am responsible for my own pet’s care and medical attention.

I have disclosed any previous or current medical issues, allergies, special dietary needs or medication needs of my dog.

I understand that although all dogs are fully supervised, that incidents of injuries may occur. If my dog falls ill or injured while in the care of Westrock Kennel and requires medical attention, I authorize Westrock Kennels to bring my dog to the vet for emergency care for appropriate veterinary assessment and treatment. I accept responsibility for all associated expenses and will pay this directly to the vet as incurred. In this circumstance, the owner or emergency contact will have to resume care of the dog upon discharge from vet hospital in order to provide appropriate follow-up care.

I release Westrock Kennels, its employees, and owners from any liabilities but not limited to injury, sickness, damage, accident, or death of my dog while under care of Westrock Kennels, on and off-site property.

I confirm that I am not aware of any vicious tendencies of my dog. My dog is sociable and has not harmed or shown threatening behaviors towards any person or any other dog. I understand Westrock Kennels reserves the right to remove my dog from any area and place my dog in a separate holding area if my dog displays any unwanted behaviors. I also understand that Westrock Kennels reserves the right to permanently remove my dog from their property at anytime.

I agree that I have discussed any behavioural concerns of my dog with Westrock Kennels. I therefore release Westrock Kennels from any liability should my dog injure another dog or person and I accept all medical and legal responsibilities and associated costs for my dog’s actions.

I agree that my dog may be recorded on camera and its images or videos may be used for Westrock Kennels promotional material.

I agree to pay the full service rates in effect for my dog’s care and participation in activities at Westrock Kennels. I further agree to pay for any additional services requested such as bird fees (prices vary).

I agree to pay for any and all outstanding charges prior to or at time of pickup of my dog. I understand that should I not have provisions for payment at the time of pickup of my dog, Westrock Kennels reserves the right to withhold my dog and continue to charge for continued services and any additional fees until payment is made in full.

In the event that I or my alternate contact does not pick up my dog on the agreed upon departure date, I hereby authorize Westrock Kennels to continue to provide boarding and any additional services at my expense.
