As an organization dedicated to empowering teens and young adults who experience developmental disabilities to lead a meaningful and inclusive life we strive to ensure that the rights of all persons we serve is defined, communicated in advance of engagement in programs and services, and to provide individuals served and their families with a mechanism to file a complaint or grievance in the event their feel their rights or the rights of their family member have been violated.

Please provide the request information below in order to allow our organization to review your complaint and work to address the circumstances.

Upon submission of the complaint form Summit Future Foundation will review this information and work to resolve the issue in a timely manner. If you elect to disclose your identity or the identity of the person you are filing a complaint on behalf of you will be notified in writing as to the organizations efforts to resolve the concerns reported within 30 days.
