Feel free to copy and modify this document in any way. I have changed a few things, to make it easier to copy for individual programs. Since I created and shared this log, I have made a few changes to the one we will use. I rearranged some of the questions to make the "Response" sheet easier to read. I also separated this into two different logs--one for "personal contact" and one for "distance learning." Even though that creates two different links and two different places, I thought that would be more user friendly and easier to enter data into KAERS.

Also, if you don't know, Google Forms can automatically populate a Google Sheet if you click on "Responses" above then "View Responses in Sheets". You only have to create the Google Sheets "responses" document once. Then future entries will automatically load into the created response sheet.
