This is a form for individuals to provide details of what they're experiencing within the association. Many parents/players/spectators have a variety of experiences while being involved with minor hockey and BDMHA would like to hear from you all.

This isn't just a form to submit negative feedback, positive feedback is welcomed.

If you are reporting negative feedback, please be constructive as we want to establish a solution to your problem or issue. Be as specific as possible and if you do not have enough space to detail out your entire message, please provide a contact eMail or Phone number that you can be reached at to discuss further.

We want to make sure that a person's information can be kept anonymous, all the form spaces are optional, you do not have to provide contact information if you do not wish to.

Finally, the association also recognizes that bullying is a very serious issue and is not to be tolerated. We encourage individuals to notify us if you or someone you know is being bullied in a hockey setting. The association wants to create a safe and fun environment for all members.
