Lincoln Lutheran School Nutrition Program: Application for Participation 2019-2020
In order for your student(s) to participate in the School Nutrition Program, please complete the following information for the school office. This form must be on file before August 13, 2018. Payment Policy 2019-2020
All student lunch accounts will be maintained using FACTS. Balances due to lunch accounts will be transferred to FACTS for payment 2 times monthly. Expenses in FACTS of $26.00 or more past due will be subject to a $25.00 late fee. There is no longer an online or pre-payment option. It is the policy of the School Nutrition Program that students will not be refused meal service. Please be sure your student brings a lunch if you do not wish them to purchase a lunch. Student lunch price is $2.90 per meal.
