There is a supply fee of $25 per child ($10 if you meet the criteria in section 4). The form will guide you through the registration process. Supply fees can be paid via PayPal, credit, or debit card.  Registration is not considered completed until this form is finished, fees are paid, and any IEP or 504 plan is submitted and approved.

If your child has an IEP or 504 plan, you will need to submit a copy of that form to rcfreedomschool@gmail.com.
