Small Training Session Consent Form (COVID-19)
Now the UK Government has given approval for outdoor competitive grassroots football and training sessions to re-start, this detailed guidance has been produced by The FA in relation to football in England. It applies to outdoor youth and adult football, as well as the small-sided game. For now, indoor football and futsal are not permitted.

With this in mind Larkhall Athletic Youth FC has decided to start offering training sessions to their respective age groups. Applicable age groups will be dependant on each Age Group Manager whether it will be offered or not. Further information can be found on the FA website: https://www.google.com/url?sa=t&rct=j&q=&esrc=s&source=web&cd=&ved=2ahUKEwjE8ILlidDrAhWUilwKHfb3CVEQFjAAegQIAhAB&url=https%3A%2F%2Fwww.thefa.com%2F-%2Fmedia%2Fthefacom-new%2Ffiles%2Fget-involved%2F2020%2Fdetailed-covid-19-guidance-on-re-starting-competitive-grassroots-football.ashx&usg=AOvVaw2cpfTqyaO_dWFVgoibRcMR

One of the requirements is to seek your consent and agreement with the rules we have outlined below. Note the coach will apply the set rules; if at anytime your child does not consistently uphold the requirements then we have the right to kindly ask you to pick up your child's soon as possible to minimise risk to others. Of course this is totally optional and you wish not to send your child to training. We anticipate normal training will start again in September - we expect the FA to publish further guidelines on this.

1. Parents to drop children off, walking them to the venue and then picking them up whilst keeping social distance.

2. Players to bring their own balls if preferred to training .

3. Any other equipment being used by coaches (IE cones , agility ladders , hurdles..etc) to be wiped down before and after the session.

4. Players to bring hand sanitizers to training and use before and after the session.

5. At sessions, the players can interact under normal football rules, no handshaking before game etc. coaches will be at least 2m apart at all times.

6. Parents can watch if they like but from a distance and to uphold social distancing

7. Injuries, the parent of the injured child will be contacted to come and collect their child. If the injury is an emergency,  then the coach will do their best to treat the injured person until further help arrives.

8. In the event the number of willing attendees goes over the maximum number restrictions, the coach will confirm who is attending that session and will then if need be rotate players the following week so it’s fair for every player.

9. Your confirmation of your child’s interest to attend a session means you agree to the above rules.
