Self-Censorship Survey B
Purpose
The purpose of this study is to explore the effects of self-censorship on an individual's socio-political outlook.  We hope to learn the emotional and ideological effects of self-censorship.

Informed Consent
It is your choice whether or not to participate. If you choose to participate, you may change your mind and leave the study at any time. If you decide to take part, you will be asked to answer a series of questions as honestly and accurately as possible. The survey should take between 5-10 minutes to complete.

Confidentiality
The members of the research team and the Huntington Beach Union High School District Institutional Review Board (IRB) may access the data.  The IRB reviews and monitors research studies to protect the rights and welfare of research subjects.

We will keep your records for this study confidential as far as permitted by law.  However, if we are required to do so by law, we will disclose confidential information about you.  Efforts will be made to limit the use and disclosure of your personal information, including research study and medical records, to people who are required to review this information.  We may publish the information from this study in journals or present it at meetings.  If we do, we will not use your name. When the results of the research are published or discussed in conferences, no identifiable information will be used.

Possible risks and discomforts you could experience during this study include:
The risk of a breach of confidentiality. There is a small risk that people who are not connected with this study will learn your identity or your personal information; however, no identifying information will be collected. As you answer the series of questions you may come across a question that makes you uneasy, you may skip any question or stop the survey at any time. There may be other risks that are not known at this time.

The investigators are required to report certain cases with the potential of serious harm to you, or others, such as suicidality or child abuse to the appropriate authorities.
