This form is to gauge interest in how many student meal packages might be distributed each week. Meals are served on a first-come, first-serve basis. Completing this form does not guarantee you will receive a meal. It is only to provide our staff with an estimate of how many meals may be needed at each school.

If a child is attending the Boys & Girls Club, they can only pick up meals they would not receive at the Boys & Girls Club.
