Team Checklist Form for Return to Play Guidelines for 2021-22 Season
All USQ teams wishing to hold in-person activities and events while the return to play guidelines for COVID-19 are in effect must submit this form. Teams can use this page to check their baseline conditions: https://www.usquidditch.org/teams/covid 

The form must be submitted no later than a week before your team’s first in-person activity or event. It only needs to be submitted once per team or joint team group.
