Ideally, the form below must be completed in full by a parent or carer of the young person hoping to join if they are under 16, or if over 16 then the participant themselves can fill this form in.

In order to give you the best experience and support, we will ask your permission to speak with a family member and/or social worker/PA for more detailed info if needed.

Please note, we have limited capacity and if oversubscribed, we will work to ensure young people most in need are prioritised for a place.
