Faith Formation Registration 2022-2023
Parents - please fill this form out as thoroughly and accurately as possible and submit by Wednesday, August 10th. As our Faith Formation Program is for the WHOLE family, please include ALL of your children on this registration, not only those preparing for sacraments this year. However, please also note that the program for 7th & 8th grade Confirmation preparation will be separate in many ways this year and will require a separate registration (stay tuned for more information). If there is information you need to share with us that does not fit into any of the particular questions or categories, please add it in the notes at the end of the form. You will also be receiving a paper copy of the registration form, or may pick one up in the Gathering Space, if you would prefer to fill out a printed form and return it to the Parish Office by the same date.
