**Before filling this out, you MUST have already completed Dartmouth's online certification module.**

To do that, submit the request form with OPO Reservations Coordinator Kate Crocker and click the link in the e-mail you get from Transportation Services. Once that is complete, fill out this form to request a vehicle training for the DOC or OPO. One of the student driver trainers will contact you to schedule your training. After you have done your on-road training(s), see the Reservations Coordinator to update your Driver Card.
