Please complete this form ONLY if you would like an instructional model change for your student beginning 4th quarter on March 30, 2021. There are two instructional model options for the 4th quarter: All-In or 100% Virtual. Once the below form is complete, the building principal will be in touch with next steps. The deadline to complete the below form for the change to be in effect on March 30, 2021 is Friday, March 12, 2021. Forms completed after this date are not guaranteed an effective change date of March 30th.
