Welcome to our enrollment form for newty families. Please fill out this form and submit your answers at the end.  Priority current family re-enrollment will end on March 13th at 12:00am. We will open registration to new families on Monday, March 14th, 2022.

If requesting tuition assistance, the separate FACTS application is required. Completion by May 8th, 2022 is optimal, if May 8th has already passed the FACTS application does still needs to be filled out by new families if requesting tuition assistance: https://online.factsmgt.com/signin/4L2TZ

New kindergarten students that did not complete St. Mary's Pre-k program are required to complete kindergarten readiness testing administered by St. Mary's staff. Please contact the office to schedule readiness testing.

Do not enroll Pre-k or TK students on this form, use the other form designed specifically for new Pre-k/TK families.
