I acknowledge I am required to complete and upload the Essay for Paramedic Application form located in CastleBranch. *
I acknowledge I am required to have a physical within the past 12 months. *
I acknowledge I am required to have both the Barton Physical Form & Barton Student Health form, located in CastleBranch,  completed by a healthcare provider, and uploaded in CastleBranch. *
I acknowledge I am responsible for getting a TB Skin Test each year. *
I acknowledge I am responsible for getting a Flu Shot each year. (Current Flu season is Aug 1 - March) *
I acknowledge I am responsible for completing my Hep B series (3 or positive titer). *
I acknowledge I am responsible for completing my MMR series (3 or positive titer). *
I acknowledge I am responsible for completing my Varicella series (2 or positive titer). This cannot be proof of chicken pox. *
I acknowledge I am responsible for maintaining and uploading my most current EMT or AEMT card. *
I acknowledge I am responsible for maintaining and uploading my most current HCP or Rescuer CPR Certification. *
I acknowledge I am responsible for uploading a NEW proof of insurance letter at the beginning of each semester. *
I acknowledge I am responsible for uploading 2 SIGNED professional letters of recommendation. *
I acknowledge if I am offered a spot in the paramedic program, after completion of application and interviews, I am responsible for completing a background check through Castlebranch at my own expense. (please do not complete this until instructed to do so) *
Pre-Application to Paramedic Program
I acknowledge I have completed Steps 1-5 of the Paramedic Pre-Application Step-by-Step Guide. *
I acknowledge and understand all transcript requirements listed on #4 on the Paramedic Pre-Application Step-by-Step Guide. Most importantly, I understand all HS & college transcripts must be Official and sent directly from the institution to Barton. *
I understand this is not the paramedic application, rather the pre-application requirements needed prior to starting the online Paramedic Application through CastleBranch. *
I understand Barton is charged each time we issue a CastleBranch paramedic application.  I do not have any further questions and I am ready to move forward. *
