We will be using your answers to order your dancer's costume(s). Form and payment must be completed by January 1st. If you have multiple children at TDS please fill this form out again. One form per child. ***SIZING/ORDERING OPTIONS*** You have the option of determining your child's size or requesting our help. Regardless of your response we will be measuring each child to minimize errors.

Please place your payment in a sealed envelope. Write your child's full name on envelope. Once we receive an envelope with your costume payment we will send you a receipt of purchase to this email address. We are only accepting cash for costumes, exact change please!

Recital times: Children registered in classes ages 3 to 9 are typically in our noon show. Children registered in classes aged 10 to advanced are typically in our 5pm show. Please keep this in mind when Choosing your classes and making your schedule. Sometimes we are unable to accommodate siblings that are in different age groups.Free tickets will not be offered
