This form is to be filled out by coach/team representative for the entire team. You will be emailed an invoice to pay online. If you need to pay in person or an alternate method, please contact Ky at kturner45@gmail.com. Players must play up a grade, but they may not play down. You may be asked to provide a current grade card. All team members must be at the same school.
