The Beresford School District expects students to ride district provided transportation to and from all district activities.  However, in the circumstances that a student needs to ride with a parent/legal guardian to or from an activity, this online document should be completed at least one day prior to the activity.  It is expected that the student will remain at the activity venue until the completion of the event.  

I, the Parent/Guardian of the student listed on this form, release the Beresford Schools and the advisors, coaches, and administrators from any responsibility for the mentioned student once that student is released to me for his/her travel to/from the activity mentioned on this form. I am aware that the student will only be released directly to me after I, parent/legal guardian, have connected with a coach/advisor at the event to verbally confirm that I am taking my student home.  (Students may not provide their own transportation to/from Beresford School District Activities events that are held out of town.)
