Monthly Membership Rates are based on hours per week, NOT classes. 
Classes may be one hour, two hours, or three hours in duration.

**We will tally your monthly hours and send you an invoice for your monthly payment after completing this form. 

Monthly Memberships are due by the 5th of each month.
Payments will be due January 5, February 5, and March 5, 2023.

1 hour per week: $70/month  
2 hours per week: $130/month
3 hours per week: $180/month
4 hours per week: $220/month
5 hours per week: $260/month
6 hours per week: $290/month
7 hours per week: $330/month
8 hours+ per week: $350/month
