Insurance/Liability
I, the undersigned, completely understand the risks involved in and do hereby consent for my child to participate in any of the activities, practices, games, tournaments, camps (“Bearcat Football Team Camp”) entered into and sponsored by the Burlingame Recreation Commission (“BRC”) and to use the facilities of the Burlingame Schools or any other facility designated as the location for the team organized activities, practices, games, and camps. I specifically consent to allow my child to participate in the 2023 Bearcat Football Team Camp, and hereby release, indemnify and hold harmless the City of Burlingame, the Burlingame Recreation Commission, Burlingame USD 454 School District, coaches, assistant coaches, members, players, agents, commissioners, employees and assigns from any and all liability, claims, actions, demands and judgements arising out of any injury or loss sustained by the above named child, myself, my family, or guests in connection with the Bearcat Football Team Camp, any team practice sessions, any team sponsored or team recognized activities, travel, or any other functions that the above-named child, myself, my family, or guests are invited to as Members of the BRC team. I have read the Burlingame Commission Waiver of Liability, understand it and voluntarily accept it. I have the authority to sign the Burlingame Recreation Commission Waiver of Liability on behalf of my above-named child.
