글로벌트라우마센터는 (재)글로벌엘림재단 산하 기관으로, 외국인 주민과 다문화 가족의 심리적 지원을 위해 최선을 다하고 있습니다.
글로벌트라우마센터는 내담자의 상담과 협력상담기관과의 원활한 연계를 위해서 아래와 같이 정보 제공 동의를 받고자 합니다. 내담자가 제공하는 모든 정보는 상담 예약과 안내, 위기상황 개입과 상담의 목적 외에 비밀이 보장됩니다.
본 신청서를 작성함은 다음과 같은 사항에 동의하는 것을 전제로 합니다.
- 상담자는 위 내용을 고지하였고, 신청자(내담자 본인 또는 보호자)는 위의 내용에 동의합니다.
* 이용 목적
- 상담 예약과 안내
- 위기 상황 개입 및 내담자 상담
* 개인정보 보유 및 이용기간
- 취득한 정보는 상담 완료일로부터 5년 간 보관 및 보존 기간 경과 시 즉시 폐기.
* 내담자는 개인정보 동의를 거부할 권리가 있으며, 동의 거부에 따른 서비스 제공 관련 불이익이 없습니다.

The Global Trauma Center seeks to obtain consent to provide the following information for smooth connection with the counseling and cooperative counseling institutions of the client. All information provided by the client is confidential, except for the purpose of counseling reservation and guidance, crisis intervention, and counseling.
Filling out this application form is premised on your agreement to the following:
- The counselor has notified the above, and the applicant (client or guardian) agrees to the above.

* Personal information retention and use period
- Acquired information is stored for 5 years from the date of counseling completion and immediately destroyed when the retention period elapses.
* The client has the right to refuse to consent to personal information, and there is no disadvantage in providing services due to refusal of consent.
