Immaculate Heart of Mary - Rite of Christian Initiation for Children Registration Form - 1st Year
This program is for children over the age of 7 who are not Baptized, and would like to receive the Sacrament of Baptism, First Reconciliation, First Holy Communion and Confirmation. 
*Please submit this form by September 5th.
This is a 2 year program.
Classes are Sundays from 11:30-12:30. See brochure for class dates this year. 
Registration fee: $50 one student, $90 two students, $100 three or more students from the same family. Please bring cash or check (made payable to IHM Faith Formation) to the office or Justine Benedict.
