Varsity Baseball Parent Travel Release Form
Please fill out this form the night before the game you will be taking your son home from.  

Please provide the DATE and LOCATION of the game you will be taking your son home from.  (Ex. 1/8/21 Cy Lakes HS).

Because of unplanned circumstances, do not fill out this form for future games in the season.  

The travel release form must be filled out the night before the game you are wanting to take your son home from.  No earlier or later.

I understand I am responsible for my son, and do not hold the Oak Ridge baseball team or the school liable for anything that happens when he is riding home with me.
