THE CHURCH RESERVES THE RIGHT TO CANCEL OR CHANGE THE 2022 VBS PROGRAM IN ITS ENTIRETY OR IN PART AT ANY TIME FOR THE SAFETY OF THE PARTICIPANTS, OR FOR ANY OTHER REASON. THE CHURCH SHALL NOT BE RESPONSIBLE FOR ANY FINANCIAL LOSS ARISING FROM SUCH CANCELLATION OR CHANGE.
I/we agree to instruct my child and to comply with the following responsibilities:
To comply with all established guidelines;
To respect and obey the instructions of Pastors-in-charge, volunteers, and other leaders;
To respect other students and participants;
To take all personal safety measures, including use of any and all necessary personal protection equipment, if and when my child participates in any VOLUNTARY activities taking place at the Church;
To inform the Church of my child’s and my household’s health or medical conditions that are associated with the symptoms of Covid 19 infection as published by the Center for Disease Control (“CDC”);
To seek medical treatment immediately if and when my child or any other member of my household shows any health or medical conditions that are associated with the symptoms of Covid 19 infection;
To respect the Church facility in the event any activities take place at the Church;
To maintain confidentiality of all personal information (e.g., names, residential address, email address, date of birth, and other non-public personal information) of all participating students, volunteers, leaders, and pastors; and
To speak with Pastors-in-Charge, leaders, or other volunteers IMMEDIATELY if and when my child is subject to online bullying or attack, or otherwise feels uncomfortable or unsafe.
I/we consent to and agree to my child participating in the VBS program. If any VBS related activities are scheduled to take place at the Church facility, I/we understand that my child is NOT required to participate in such VBS activities at the Church facility; and if my child does participate in such activities, I/we understand that there may be certain health and medical risks that the Church cannot guaranty against. I/we recognize that the Church, the Pastors-in-Charge, or the volunteers cannot be held personally responsible for conditions beyond their reasonable control, for my child’s breach of his/her responsibilities, or for a third-party’s breach of their responsibilities, and I/we agree not to hold the Church, its staff, volunteers from any and all personal liabilities for any medical or personal exposure, injury, loss, or damage to person or property that may occur during the course of my child’s participation in the VBS program.

I/We hereby grant to the Church permission and rights to record, play, perform, publish, and/or use recording, singing, performance, and portraits or pictures of my child, in whole or part (“Recording”) in any form or forum, including in CD, DVD, on Website, and in whatever electronic and/or digital media, and without further compensation, for the Church’s Sunday School ministries, including Vacation Bible School (“VBS”). I/We further authorize, assign, transfer and convey any and all copyright to said Recording to the Church.
