This form is for you to nominate to be part of 2020 HAW Capital League Spitfires.

By submitting this form and trialing, the player nominated (and if under 18, their parent/carer) agrees to abide by the Hockey Australia Code of Conduct which is part of your Hockey Victoria registration.  You are also committing to be a financial member with Hockey Victoria in 2020.  

For all players under 18, all emails will be sent to the parent/carer, copy to the player.

Training and squad selection dates will be advised via email.

This form must be completed and submitted before December 22 2019. Any issues please contact the HAW office.
