RMC Party Registration and 2018-2019 Evaluation
For RMC members, please fill 1 form per student since the feedback is for the coaches of your group.

For non RMC members, please fill 1 form per family.

Please register for the RMC Year-end Celebration: May 29th, 7:00-8:30 PM at Harwick Cafeteria of Mayo Clinic.

We will have Art on the Go as a group activity. This part costs $22 per student. RMC will pay $7 and please pay $15 per student for this activity.

We would like to also collect your feedback on our activities during the 2018-2019 school year, and your suggestions for the 2019-2020 school year. Thank you for supporting RMC!
