A very warm welcome to Surrogacy Australia. Thank you for starting this application form to join the organisation as an Intended Parent or parents.
At Surrogacy Australia’s Support Service (SASS), you will benefit from a wealth of experience from many members within the Australian Surrogacy community, including access to an experienced Intended Parent (now a parent!) who will be your Mentor at the start of your surrogacy journey. Our ethos is ‘Friendship First’ and we feel that it is an important part of any surrogacy journey.
This application form is for Intended Parents (IPs) who HAVE found a surrogate. If you're joining SASS and have NOT found a surrogate, there is a different application form to complete.
If you're completing this form, you already have found a generous woman to be your surrogate. Congratulations! 🎉 Congratulations to you all for your commitment to becoming a surrogacy team and for investing this time into educating yourselves about surrogacy. As we say, surrogacy is a marathon not a sprint and teams that invest this time in the early days tend to be the teams that have more positive journeys overall.
We recognise the connection you already have together, be that a connection of many years or one that is relatively new. You're embarking on a very intimate journey as you make a baby together. Thank you for allowing us to share some education on how to make that shift in your connection and to being open to recognising a deepening that will happen between you all.
The questions on this form serve several purposes. First, we need to obtain some basic information about you for our records. It will also help when assigning you with a Mentor and to guide the conversations to be had during that session.
Second, it helps to educate you about some of the important expense elements and steps in surrogacy to assist in your planning.
Finally we ask some questions about your hopes and expectations for surrogacy. Don’t worry if you have not reached definitive answers to these questions. We include them here so you can start thinking about these issues, and you can discuss them further at your face-to-face Mentor session.
In summary, we want to educate Intended Parents to make sure they:
* are eligible for surrogacy due to medical or social infertility
* are financially ready to undertake surrogacy
* and that altruistic surrogacy is the right choice for them
We do ask you to fill the forms in as accurately as possible. If you feel there is not enough space to provide the full answer you would like to give, please email us with your extended response. Please note there are no right or wrong answers – we consider all applications and circumstances. What is important is to be as honest as possible as this will make sure that we can properly support you on your journey. If there are any parts of the form that you are unsure about, please get in touch and we can help you through every step of the application process. If you have deliberately withheld information, or have given false information, we may cancel your membership. In this case a refund of your membership fee will not be possible.
