Use this form to request a refund for purchased KdVSF Summit Tickets. 
Refund requests received by June 16 will receive 100% refund. 
Requests received between June 17-June 30 will receive 50% refund. 
No refunds will be issued after July 1. 

Refunds will be issued directly from the KdVS Foundation.
