The purpose of this application is to ensure financial assistance is awarded in a consistent and relevant manner. This application pertains to participation in the Lake Cities Girls Softball Association. ALL INFORMATION GIVEN TO THE LCGSA SCHOLARSHIP REVIEW COMMITTEE IS CONFIDENTIAL.

3. Criteria:

1. Participant(s) and/or siblings in the household MUST NOT be playing in any “Select” program, regardless of sport.

2. Must disclose any other scholarships family received within the past 2 years from any Lake Cities (Lake Dallas, Corinth, Hickory Creek, Denton, and Shady Shores)

3. Agrees to participate in the majority of practices and games (failure to do so may impact the participants ability to qualify for future scholarships)

4. Family must demonstrate clear financial need due to an emergency and/or recent financial change

5. Application must be turned in by the deadline to the LCGSA Softball Board for review and subsequent approval or denial.

4.  The following documents are required for consideration if applicable:

1. Completed Scholarship Application.

2. Most recent pay check stub representing pay for the last 30 days.

3. Aid to Dependent Children.

4. Copy of birth certificate.

5. Free/reduced Lunch letter from the current or previous school year.

6. Food stamps documentation, or Public Housing documentation.

1. Supporting documentation MUST be attached to this form for scholarship consideration
