Ages 7-11. Camp size limited to 12. All forms and fees due July 14, 2022 to attend camp. Please read, sign and date: I/We the parent(s)/guardian(s) of the above-named child hereby give my/our approval for his/her participation in any Bass Harbor Memorial Library Summer Arts & Nature Camp activities.I/We assume all risks and hazards incidental to such participation and do hereby waive, release, absolve, indemnify, and agree to hold harmless the Bass Harbor Memorial Library, the organizers, sponsors, supervisors, participants, and persons conducting those activities, from any claim arising out of injury to my/our child.In the event, I/we cannot be reached within a reasonable amount of time, I/we give permission for Camp personnel to contact the above-named emergency contact with questions concerning my/our child.I/We further consent to and authorize the reproduction for publication and use by the Bass Harbor Memorial Library and their successors and assigns, for advertising, commercial and promotional use and any other purpose, any photos, pictures, or likenesses of myself or my child. I understand registration is required to secure my child's spot at camp. If I do not register or pay in full by July 14, 2022, I understand my child may lose their spot at camp.
