Middle School Community Assessment
Thank you for your consideration in completing this survey. We are a group of students pursuing Bachelor and Master degrees in Social Work, Public Health, and Communications at the University of Hawaii at Manoa. We are supporting Atherton YMCA programs and services and are interested in exploring how COVID-19 has impacted middle school students.

This survey is for students in middle school and is completely anonymous. No personal information collected in this survey will allow us to identify any survey participant. This survey is optional and there are no incentives to participate. We encourage all students to secure their parent's or guardian's consent before participating. Participants may discontinue taking the survey at any time. At the end of the survey, there will be a link to a variety of community resources to support students' social, academic, and emotional needs.

This assessment asks students to briefly share their experiences, struggles, successes, and needs through COVID-19 relating to academic performance and mental well-being. The sudden shift in the world around us and in our daily lives has created or exacerbated challenges, and we aim to better understand these changes. The results of this survey will help the YMCA better understand how young people have been impacted, how they are coping and what they may need to allow the YMCA to explore and create new opportunities to continue to support teens in upcoming Spring programs.
