Dear Rockwood families,

Please complete the following April Pivot  Chromebook Request Form only if you do not currently have a UGDSB compatible device at home. At this point, we can offer one chromebook per family. Please fill out one from per child.

Devices will be available for pick up on a daily basis and you will be contacted by email about specific pick up dates and times after your form is processed. Please only pick up your device once you've received and replied to your confirmation email. Be sure to follow the instructions in the confirmation email.
