This form is for teachers/staff to fill out for a facility request that pertains to  school needs.
Use this form when reserving one of the district facility building/areas listed in the form below.
If more than one building is required please fill out a separate form for each one
Please allow 72 HOURS for APPROVAL of this request.
Upon approval it will be posted on the facility calendar

A large trash can will be placed in the  area for you to us. Please help us leave the area as clean as possible once the event is over.  The custodians will take care of “normal” cleaning of the bathrooms, please do a quick walk through to make sure there are not any extra messes left by your attendees. *

All lead custodial staff, of the building you are requesting, will receive a copy of your request to know what you are wanting for set up. *

At this time we are focused on keeping the school buildings opened and safe for our students and staff. We are very short handed on support staff which is why we are limiting the days to Monday-Thursday *
If the building is closed the facility is closed. If your event will be cancelled, you will be notified by email of the cancellation. (No school due to  inclement weather or quarantined or the building principal has something scheduled for example) *

If your event has not been approved and on the facility calendar, climate control, cleaning/preparation and lighting will not be in place. The custodial or event staff cannot make exceptions. Please verify your event is on the calendar before it is scheduled to take place. *

The building principals have the right to deny you access to the building for your event. *

By signing this document you are agreeing to the terms and expectations listed above. Failure to comply will result in either removal from the facility or cancellation or denial of future requests. *

We will do our best to keep that date and time for you, but please understand that school related events take priority, if there is a conflict you will be notified ASAP to reschedule.                                                          We the undersigned hereby makes application to Camdenton R-III School District for the use of school premises and certifies that the information given above is correct. The applicant will exercise the utmost care in use of the school premises and property and will make good on any damage arising from the applicant’s use of said premise and property. We hereby agree to defend, indemnify and hold harmless the Camdenton R-III School District, Board Members, administrators, teachers, employees,  volunteers, and agents from and against any and all liabilities, claims, damages, penalties, actions, suits, losses, costs, and expenses arising out of or in connection with the use of said premises. This agreement to defend, indemnify, and hold harmless specifically includes all claims arising out of any accident or other occurrence on or about said premises causing injury to any person or property. The undersigned has read and is in agreement with the rules and regulations for building. *
